When you book your first visit, you will receive your new patient paperwork via email that can be filled out electronically. If this option is not accessible to you, you may print the forms below to bring with you. Please arrive a few minutes early if you need to fill out the forms in-office.

       The first step is to take a full history of your chief complaint and health status, which will help determine if chiropractic care is right for your current condition. If our services match your needs, we will then perform a comprehensive physical exam to get a better understanding of your chief complaint. In some rare cases diagnostic imaging may be necessary for more information before treatment. If so, you will be referred out for those to be completed at that time. If no further investigation is necessary, we will then discuss your condition and our plan of action for treatment. This will be followed by your first treatment; including adjustment and/or any ancillary treatments appropriate for your condition at that time.

  GRC is a "self-pay" clinic, which means we do not accept any insurance. This also means we are not taking any new patients who are Medicare eligible at this time.
